What you should know before we begin
The NYC Health and Affordability Connector connects you to services you qualify for to lower the cost of living.
To simplify the process of enrolling in multiple services, the NYC Department of Health and Mental Hygiene (“NYC Health”) will collect and save your full name, date of birth, address, and contact information.
Our partners will help you get to the finish line. If you pick an NYC Health partner organization’s services, we will also share your information with them so you don’t need to re-enter it. Your activity with our partners will remain linked to your account on this platform.
How we use your information
To best serve you, we will track your enrollment progress and contact you with reminders by email, phone, or text. You may opt out at any time.
If you choose to use a partner organization’s services, we will share the information they need to help you. Some partners may ask you to read and agree to their own privacy rules before they assist you.
NYC Health may use data from this platform and our partners to study, evaluate, and improve the program. All of your data will be handled in a secure manner. Anything we share publicly will not include identifying information.
I agree.

Do you have an unaffordable hospital bill?
This includes bills from providers who treated you during your hospital visit, like radiologists, surgeons, or ER doctors.
NYC Health partners with Dollar For and the Community Service Society of New York (CSS) to help you apply for hospital financial assistance. Your hospital decides if you qualify.
Tell us about your bill.
If you have more than one bill, use the most recent one.
The hospital directory has not been set up yet. These are the built-in New York City hospitals, not the full list — a bill from any other hospital cannot be recorded yet. Tell your site supervisor.

The record is unaffected. The client profile still exists, the intake still counts, and any other pillars they selected continue as normal. The client can be screened again later if their situation changes.

Let’s see if you qualify for cash back from the IRS.
Tax credits like the Earned Income Tax Credit (EITC) give working people cash back. You may qualify even if you don’t earn much, don’t usually have to file taxes, or have never filed before. Your chances are even better if you take care of children.
Missed out in the past three years? You can still file or amend past returns to get that money back.
Answer a few quick questions to find out if there are tax credits or a refund you can claim.
Have you filed taxes in the last three years?
Did you have income from work in the last three years?
Do you have children that you care for?
